Dear 5th Grade Families,
June 8th, 2026 is our 5th grade senior trip to Dave & Busters. Four (4) parents from each class will be chosen to attend the trip. If you are interested in being a chaperone please complete the form below and return it to your child’s teacher no later than May 22, 2026. We will be selecting chaperones randomly, if you are selected you will be contacted by the PTA. The adult price is $20 (buffet lunch) and will have to be paid by June 4th, 2026. If payment is not made on time, the PTA will select another parent to attend.
**Payment must be made by CASH. Please DO NOT send in payment until you have been selected.
